Healthy hair follicles, usually taken from the nape area, are transplanted to the areas of hair loss to achieve a natural appearance. The procedure is performed under local anesthesia and is a minimally invasive technique.

The FUE hair transplant method is based on placing individually harvested grafts using special micro instruments. Since this method does not require stitches and does not leave scars, it is one of the most preferred applications among modern hair transplant techniques. The recovery process usually takes a few days.

The DHI hair transplant technique is performed by directly implanting grafts with special implanter pens. This method provides more control over hair density and direction. It is particularly suitable for individuals who want natural and aesthetic results in frontal hairline design.

The post-operative care process is of great importance for the success of the hair transplant. The first wash, the crusting period, and the process of the hair follicles anchoring should be followed meticulously. Adhering to expert recommendations ensures that the transplanted hair grows healthily and that permanent results are achieved.

 Resume

 Make an Appointment

Prof. Dr. Şule Güngör
Professor of Hair Transplantation and Dermatology

Prof. Dr. Şule Güngör is an academic and physician with over 20 years of experience in dermatology and medical aesthetics. id="What_are_the_fundamentals_of_hair_transplantation_and_how_have_developments_progressed_in_this_field">What are the fundamentals of hair transplantation and how have developments progressed in this field?

The basic rationale behind hair transplantation is actually quite simple and is based on a scientific principle called “Donor Dominance.” This principle states the following: Hair follicles in certain areas of our body are genetically resistant to the hormonal effects that cause hair loss. The hair on the nape of our neck has this characteristic. When we take these hair follicles and transplant them into a balding area on the top or front of the head, the follicles do not forget their original genetic code. In other words, they continue to be resistant to hair loss and to grow for a lifetime in their new location. It is just like uprooting a hardy plant from one corner of your garden and planting it elsewhere, and the plant continues to grow with the same resilience in its new spot. This principle is the scientific proof of why hair transplantation is not a temporary camouflage but a permanent solution.

The journey in this field started far from today’s natural results. In the techniques that began in the 1950s and continued for many years, very large tissue pieces (punch grafts) were used. This method caused a clumpy and quite artificial appearance, popularly known as “grass man” or “doll hair.” At that time, this was the only option available and many people had to accept these unaesthetic results.

Fortunately, as medicine and technology advanced, so did the aesthetic understanding. In the 1980s and 90s, the use of smaller tissue pieces called “mini” and “micro” grafts marked the first steps toward more natural results. The real revolution came with the understanding that hair does not grow individually but in natural groups of 1, 2, 3, or 4 called “follicular units.” This discovery fundamentally changed the philosophy of hair transplantation. The goal was no longer just to fill gaps, but to mimic this natural grouping of hair to create a completely indistinguishable, natural appearance. Today’s modern FUT and FUE techniques are built upon this understanding. Therefore, the historical development of hair transplantation is the story of a transformation from a crude covering procedure to an art form that respects the natural architecture of hair. This knowledge is valuable for alleviating patients’ fears about older methods and for understanding how natural results can be achieved today.

Who is considered a suitable candidate for hair transplantation?

Hair transplantation is not a magical solution for everyone experiencing hair loss. The success of the operation is closely tied to selecting the right candidate. Therefore, the initial consultation is a comprehensive evaluation process aimed more at determining whether the person is suitable for the procedure than at creating a surgical plan. The most common and ideal situation is “Androgenetic Alopecia,” that is, male or female pattern hair loss due to genetic and hormonal factors. This type of hair loss usually progresses in a specific pattern and preserves the hair in the nape area, providing us with a reliable source of healthy follicles.

However, not all hair loss is of this type. An ideal candidate is expected to have certain basic characteristics. These features directly affect both the success of the operation and the satisfaction with the resulting outcome.

The basic characteristics of an ideal hair transplant candidate are:

The type of hair loss (male/female pattern) being clear

The hair loss process being largely stabilized or slowed

Having sufficient hair density and quality in the nape and sides, known as the donor area

General health status not posing an obstacle to a surgical procedure

Having realistic expectations about what can and cannot be achieved with hair transplantation

On the other hand, some conditions can be a definite barrier to hair transplantation. At the forefront of these is “Diffuse Unpatterned Alopecia” (DUPA). In this condition, hair loss is not limited to the top and front areas but also affects the hair in the nape and side areas that we plan to use as donors. In other words, the region we consider “safe” is actually not safe. Since the hair follicles taken from these areas also tend to fall out, the transplant will not yield a permanent result. Additionally, individuals with active alopecia areata or scarring scalp diseases are not suitable candidates for hair transplantation until these conditions are brought under control. Finally, people whose desired hair density is mathematically impossible to achieve with their available donor hair—i.e., those with unrealistic expectations—are not considered among the right candidates to prevent post-operative disappointment.

Why is preoperative evaluation so important for a successful hair transplant?

If we liken a hair transplant operation to constructing a building, the comprehensive evaluation and planning performed before surgery is the architectural project and ground survey of that building. The sturdier the project, the more successful and lasting the structure will be. Therefore, the consultation phase is as critical as the operation itself. At this stage, the patient’s surgical “capital,” that is, the total number of healthy grafts that can be used over a lifetime, is determined and how to use this capital most efficiently is strategically planned.

Many factors are scrutinized during this evaluation process. We do not only count the hair in the donor area; we also take into account the character of the hair, its quality, and environmental factors. This holistic approach allows us to design the most natural and aesthetic result customized to the individual.

The main criteria considered during the evaluation are as follows:

Donor area density (number of follicular units per square centimeter)

Hair shaft thickness (thicker hair provides better coverage with fewer grafts)

Hair color (less contrast between hair and skin color creates a fuller appearance)

Hair texture (wavy or curly hair provides better camouflage than straight hair)

Scalp laxity (especially important for comfortably harvesting and closing the strip in the FUT technique)

The patient’s age and the risk of future hair loss progression

The most artistic part of this evaluation is hairline design. A well-designed hairline frames the face and is the most important element ensuring a natural-looking result. A mistake here can negatively affect the outcome of the entire operation. The goal is not only to create a hairline that looks good under today’s conditions, but also to design an age-appropriate plan that will not look out of place 10–20 years down the line, considering the person’s future appearance. Creating a very low and sharp hairline in a young patient can look quite artificial as hair loss continues with age and means wasting valuable donor resources. Therefore, an experienced approach always prioritizes naturalness and conservatism. This detailed planning ensures the wise lifelong management of the patient’s limited donor resource.

Aesthetics and Dermatology at International Standards

You can contact us to make an appointment. The goal of both techniques is to take healthy hair follicles and transplant them to the area where they are needed. However, there are fundamental differences in how these follicles are harvested. The choice of technique depends on many factors such as the patient’s hair structure, degree of hair loss, lifestyle, and expectations. Rather than saying that one technique is “better” than the other, it is essential to determine which technique is “more suitable” for which patient.

The FUT (Follicular Unit Transplantation) technique is also known as the “strip method.” In this method, a thin and long strip of skin containing thousands of hair follicles is surgically removed from the nape area. The area from which the strip is taken is then closed with aesthetic stitches. When healed, this stitch leaves a fine, linear scar hidden under the hair. The removed skin strip is then dissected into individual follicular units—the natural groupings of hair—by an experienced team working under special microscopes. This meticulous dissection minimizes the risk of damage to the follicles.

The advantages and disadvantages of the FUT technique are as follows:

Advantages

Allows harvesting a very high number of grafts (3000–4000 and above) in a single session.

Since follicles are dissected under direct microscopic vision, the risk of damage (transection) is very low.

Because the harvesting stage is shorter, the cost per graft is generally more economical than FUE.

There is no need to shave the entire donor area; only the area where the strip will be taken is shortened.

Disadvantages

Leaves a permanent linear scar in the nape area. This scar may be noticeable when the hair is worn very short (shorter than 1 cm).

The recovery period is longer compared to FUE and may involve more discomfort.

Tightness and temporary numbness in the donor area are more common.

In the FUE (Follicular Unit Extraction) technique, hair follicles are harvested one by one. In this method, the donor area is usually shaved very short. Using a special micromotor (or manual punch) with a tip diameter of 0.7–1.0 mm, the surgeon isolates each follicular unit from the surrounding tissue and extracts it individually. This procedure leaves, instead of a linear scar, diffuse, small white dots in the donor area that are generally unnoticeable.

The advantages and disadvantages of the FUE technique are as follows:

Advantages

Does not leave a linear scar, providing the freedom to wear hair very short.

The recovery process is faster and more comfortable. Post-operative pain and discomfort are minimal.

Since it does not depend on scalp laxity, harvesting can be performed from a wider area.

If needed, follicles can be harvested not only from the scalp but also from other body areas such as the beard or chest.

Disadvantages

Since each graft is extracted individually, the procedure time is much longer than FUT.

As it requires more effort and time, the cost per graft is generally higher.

Because follicles are extracted “blindly” from under the skin, the risk of follicle damage is higher, especially in inexperienced hands.

In cases where a high number of grafts are needed, the operation may need to be split over two days and the donor area must be shaved extensively.

Are new technologies like DHI and Robotic hair transplantation really better?

New names and technologies are constantly emerging in the field of hair transplantation, which can be confusing for patients. Especially DHI and Robotic Hair Transplantation are often marketed as the “newest” and “best” methods. However, it is important to correctly understand what these technologies mean, how they differ from traditional methods, and whether they truly provide a superiority.

DHI (Direct Hair Implantation) is not actually a brand-new hair transplant technique in itself. Rather, it is a different method used during the implantation stage of grafts harvested by the FUE method. In traditional FUE, the doctor first opens small channels in the recipient area and then the medical staff places the harvested grafts into these channels one by one. In the DHI method, a special device called a “Choi Pen” or “implanter pen” is used. There is a hollow needle at the tip of this pen. The graft is placed inside this needle. When the doctor inserts the pen into the skin, they both create the channel and, by pressing the plunger, place the graft into the channel at the same time. In other words, channel opening and implantation are performed in a single step.

It is marketed that DHI reduces the waiting time of grafts outside the body, involves less handling of the follicles, and therefore offers higher survival rates. It is also said to facilitate unshaven implantation among existing hair. While these seem like logical advantages in theory, robust comparative scientific studies proving that DHI yields superior results to the traditional channel-opening method are still insufficient. Ultimately, whether with a pen or forceps, the success of implantation largely depends on the experience, manual skill, and artistic ability of the doctor and the team performing the procedure. DHI can be an effective tool in the right hands, but it is not a guarantee of success on its own.

Robotic Hair Transplantation is a more pronounced technological innovation. This system (for example, the ARTAS robot) is designed to automate the most tiring and repetitive part of the FUE technique, namely graft harvesting. The system scans the donor area using high-resolution cameras and artificial intelligence algorithms to identify the healthiest follicles and their exit angles and directions. Then a robotic arm extracts the grafts one by one with precision based on these calculations. The robot’s biggest advantage is that it does not get tired and maintains the same precision throughout the procedure. This can reduce human error, especially during long operations.

However, robotic systems also have significant limitations. First, the cost of this technology is very high, which significantly increases the price of the operation. The robot may have difficulty recognizing blonde, white, or very curly hair. Most importantly, the robot only performs the harvesting task. The most critical and artistic stages—designing the hairline, opening channels, and implanting grafts to achieve a natural look—are still performed entirely manually by the doctor and the team. The robot does not possess the instant decision-making ability, tissue feedback, and aesthetic foresight of an experienced surgeon. Consequently, robotic surgery should be seen not as a revolutionary method but as an advanced tool that a surgeon may use. What determines the final result is not the technology used, but the knowledge and experience of the team managing that technology.

Do supportive treatments like PRP help improve hair transplant results?

Various adjunct treatments are used to maximize the success of a hair transplant operation and to support the healing process. Among these, the most popular and most studied is PRP (Platelet-Rich Plasma). PRP is not a magical formula; it is a biological method that benefits entirely from the healing power of the person’s own blood.

The procedure is quite simple: A small amount of blood is drawn from the person and spun at high speed in a special centrifuge device. This process separates the components of the blood and allows us to obtain a plasma layer rich in platelets. Platelets are natural reservoirs that, when there is an injury in our body, initiate healing and release hundreds of growth factors that enable tissue repair. The purpose of PRP treatment is to deliver these growth factors to the area where the hair follicles are located at a much higher concentration than normal, thereby revitalizing the tissue there.

During a hair transplant, PRP is generally used in two different ways. The first is adding it to special solutions in which the harvested hair follicles are kept before implantation. This helps the grafts maintain their vitality and health during the time they remain outside the body. The second is applying it directly to the recipient area through small injections during or immediately after the implantation process. This application has many potential benefits.

The potential benefits of PRP treatment in hair transplantation are as follows:

Increasing the rate of grafts (transplanted hair follicles) anchoring and surviving in their new location

Accelerating wound healing in both the donor and recipient areas

Alleviating or shortening the often-seen, temporary “shock loss” phase after surgery

Revitalizing and thickening the existing but weakened and thinned natural hair in the transplanted area

Helping newly transplanted hair transition from the resting phase to the growth phase more quickly

Many studies have shown that adding PRP to hair transplantation can have positive effects on outcomes. It has been observed that hair begins to grow earlier, grows more healthily, and the overall perception of density increases in patients. However, since there is not yet a complete standardization regarding PRP preparation and application protocols, the effectiveness of the treatment may vary depending on the method used. In conclusion, although PRP is not a mandatory guarantee of success for hair transplantation, it is considered a valuable supportive treatment that, when applied correctly, helps improve results and make the recovery process more comfortable.

Aesthetics and Dermatology at International Standards

You can contact us to make an appointment. The points to be considered during this period aim both to minimize the risk of bleeding and to ensure that the body and scalp are in the most prepared state for the operation. Adhering to these rules is a responsibility that directly affects the success of the operation.

It is critically important to discontinue the use of certain medications and supplements in the last one or two weeks before surgery. Most of these substances can thin the blood and increase the risk of bleeding during and after the operation.

The following should be discontinued 1–2 weeks before surgery:

Blood thinners (such as Aspirin, Coraspin, Plavix, Coumadin—strictly by consulting your doctor)

Painkillers in the non-steroidal anti-inflammatory (NSAID) group (medications containing Ibuprofen, Naproxen)

Vitamins B and E

All multivitamin supplements (they usually contain vitamins B and E)

Herbal teas and supplements with blood-thinning effects (Ginkgo Biloba, Ginseng, green tea, garlic tablets, echinacea, etc.)

Smoking (narrows blood vessels, slows healing, and negatively affects graft survival)

Alcohol (increases bleeding risk; should be completely stopped at least 3 days prior)

There are also some simple but important points to consider on the day of the operation. Starting the morning of the surgery relaxed contributes positively to the process both physically and mentally. You should not apply any products such as gel or spray to your hair that day and should arrive with clean hair. When coming to the procedure, choose comfortable clothing that will not touch your head after the operation and will not cause difficulty when putting on or taking off—prefer front-buttoned or zippered attire. Lastly, it is recommended to have a light breakfast before surgery, but you should definitely not consume coffee or other caffeinated beverages as they can increase blood pressure and bleeding tendency. These simple preparation steps will help the operation proceed more comfortably and safely.

What is the recovery process after a hair transplant and what should be paid attention to?

The success of a hair transplant operation depends as much on how carefully the patient follows post-operative care rules as on the performance of the surgical team. The first 10–14 days are especially critical for the transplanted hair follicles to anchor in their new locations. During this period, the grafts are quite sensitive and even the slightest mechanical trauma (friction, impact, scratching) can cause the follicles to dislodge or be lost.

The first few days are the most important stage of healing. Usually, for the first 2–3 nights, it is recommended to sleep with a few pillows under the head or in a slightly upright position in a chair. This helps minimize the amount of swelling (edema) that can occur on the forehead and around the eyes. The donor area is usually covered with a bandage, which is removed the next day. Pain is generally minimal and can be easily controlled with simple painkillers.

One of the most important issues after the operation is washing the hair. The first wash is usually performed 1–2 days after the operation and the method is demonstrated in detail. Afterwards, the patient should apply this special washing procedure every day for about 10 days. During washing, the water pressure should be very low and the temperature lukewarm. The shampoo (usually a specially recommended medical shampoo) should not be poured directly onto the hair but lathered in the hand and gently applied to the transplant area with a tapping (dab-and-release) motion. Rubbing or scratching with nails must be strictly avoided. Rinsing should again be done with low-pressure water, and drying should be carried out by gently patting with a soft towel. At the end of about 10 days, when the crusts in the transplant area have completely shed, the grafts are securely anchored and a normal washing routine can be resumed.

Activity level should also be increased gradually during the recovery period.

Post-operative activity restrictions are generally as follows:

First 3–5 days: Full rest, avoid any activities that increase blood pressure (bending, heavy lifting, sports)

Week: Only light-paced, non-sweat-inducing walks

Week: Brisker walks and very light, non-strenuous exercises

Weeks 3–4: Gradual return to normal sports and exercise routine (excluding heavy fitness)

First 1 month: Strictly avoid activities that may damage the grafts and carry a risk of infection (pool, sea, sauna, Turkish bath).

Frequently Asked Questions

Who is the best candidate for a hair transplant procedure with long-lasting results?

Hair transplant is most suitable for individuals with stable hair loss and sufficient donor hair.
