Nasolabial filler is an aesthetic method applied to reduce the lines extending from the side of the nose to the corners of the mouth. By using hyaluronic acid-based fillers, the skin becomes smoother and a younger appearance is achieved. The procedure is fast and effective.
In the treatment of nasolabial folds, treating only the fold is not enough; first, the sagging midface should be lifted with filler. Then, if necessary, filler is applied to the nasolabial fold.
In mild wrinkles, finer fillers are used to achieve a softer and more natural effect. This method supports aesthetic harmony while preserving facial expressions. Its effect is visible immediately and lasts for a long time.
Mild swelling or redness may occur after nasolabial filler, but it disappears in a short time. The duration of permanence generally varies between 12 and 18 months. With regular applications, longer-term results are possible.

What Is Nasolabial Filler?
To understand nasolabial filler, it is first necessary to know why these lines form. Although they are often called “smile lines,” these grooves do not form only from smiling. In fact, they are one of the most natural and earliest signs of the face’s aging journey. When we think of our face as a whole, nasolabial lines are one part of that whole, and their deepening is not due to a single cause.
Over time, the bone structure in our face, especially the support tissues around the sides of the nose and cheekbones, gradually weakens. At the same time, the fat pads that gave our cheeks their youthful fullness and vitality diminish and shift downward with the effect of gravity. Our skin also loses collagen and elastin, its most valuable structural components, and with them its elasticity and firmness. When all these changes come together, the cheek tissue sags downward and creates a shadow and depression along the line extending from the side of the nose to the corners of the mouth. This depression is the nasolabial fold.
Nasolabial filler is the process of restoring this lost volume. Safe and biocompatible substances such as hyaluronic acid, which is also naturally found in our body, are injected under the skin. This filler material acts like a cushion in the collapsed area, lifting the tissue, reducing the depth of the line, and eliminating the shadowing it creates. In other words, this procedure is not a simple wrinkle-filling action, but an anatomically based restorative and rejuvenating treatment that returns structural support to the midface.
What Does Nasolabial Filler Do?
When planned and applied correctly, nasolabial filler creates a noticeable and positive change in the overall facial expression. It does not simply soften a line; it also contributes to the harmony of the face. The main benefits of the procedure are as follows:
- Eliminating a tired and sad expression
- Supporting the midface
- Providing a younger and more dynamic appearance
- Improving skin quality and moisture
- Softening asymmetries in the face
- Eliminating shadows and depressions
- Creating a natural lifting effect
All of these benefits are achieved without changing the person’s own facial features, but simply by reversing the negative effects brought by time. The aim is not to look like someone else, but to look like a more rested and refreshed version of yourself. In this way, it helps create a more positive and energetic impression in social life.
How Is Nasolabial Filler Performed?
Nasolabial filler application is a procedure that requires precision and expertise, yet it is quite comfortable and quick for the patient. The process is planned from beginning to end with patient comfort and safety as priorities.
The process always begins with a detailed medical examination and consultation. In this first and most important step, your facial anatomy, skin structure, and the loss in your bone and fat tissues are evaluated. The main cause of your nasolabial line is analyzed. Perhaps the issue is not only in that line, but in the cheeks that have lost their support. Based on this analysis, a treatment plan specific to you is created, your expectations are listened to, and the targeted result is clarified.
On the day of the procedure, the treatment area is first thoroughly cleaned with an antiseptic solution. Then, so that you do not feel any discomfort during the injection, an anesthetic cream is applied to the area and the skin is allowed to numb for a while. Since most fillers used today also contain a local anesthetic within the product itself, the procedure is almost painless.
Once the skin is numb, the physician begins to inject the filler material into the strategic points determined beforehand. At this stage, one of two different tools is generally preferred: a needle or a cannula. While needles are used for very precise and targeted injections, a cannula is a flexible tool with a blunt tip. It enters through a single entry point and glides safely under the skin without harming vessels and nerves, distributing the filler more homogeneously over a wider area. For this reason, the use of a cannula is often preferred in sensitive areas such as the nasolabial region because it minimizes side effects such as bruising and swelling, as well as more serious risks.
While placing the filler under the skin, the physician uses different techniques according to the needs of your face. After the injection is completed, a gentle massage may be performed to help the filler settle into the tissue. The procedure is completed in approximately 15–30 minutes, and you can return to your daily life immediately afterward.
What Materials Are Used in Nasolabial Filler?
The material used in nasolabial filler treatment is just as important as the application technique for the success and safety of the treatment. The choice is made according to the person’s skin structure, the depth of the problem, and the desired result. There are main types of fillers used:
- Hyaluronic Acid-Based Fillers
- Biostimulatory (Collagen-Stimulating) Fillers
Hyaluronic Acid (HA)-based fillers are considered the gold standard. This substance, which is naturally found in our body, provides moisture and volume to the skin. Their biggest advantage is their safety and, most importantly, the fact that they can be completely dissolved with an enzyme called “hyaluronidase.” This reversibility provides a major safety net in case of a problem or an aesthetically undesirable result. Different HA fillers are designed for different purposes.
- Dense fillers with high lifting capacity
- Soft fillers with a more fluid structure
Biostimulatory fillers work through a different mechanism. These fillers do not only provide volume, but also stimulate the skin’s own collagen production. In other words, instead of passively filling the skin, they actively encourage it to produce healthier and firmer tissue. Their results appear more slowly but are generally longer-lasting.
- Calcium Hydroxyapatite (CaHA)
- Poly-L-Lactic Acid (PLLA)
Is It Correct to Fill Only the Line in Nasolabial Filler?
This question forms the basis of modern aesthetic understanding, and the answer is definitely “no.” The old “see the line and fill it” approach, which was common years ago, has now been abandoned. This is because it could lead to unnatural, puffy, and artificial facial expressions. Masking only the symptom instead of addressing the source of the problem does not provide satisfactory long-term results.
The real culprit behind the deepening of the nasolabial line is usually not the line itself. The problem often starts higher up, in the midface. With age, the support of the cheekbones weakens and the fat pads in the cheeks lose volume and sag downward. Just as cracks form in the walls of a building weakened at its foundation, this volume loss in the midface causes the tissues in the lower face to pile downward and deepen the nasolabial groove.
For this reason, the current and correct approach is to consider the face as a whole. The treatment plan generally follows these steps: first, the cheek and cheekbone area is evaluated. If there is volume loss and sagging in this area, the lost support is first restored to these areas with denser, lifting fillers. This gently lifts the midface upward, causing the nasolabial line to soften on its own. In many cases, this first step alone creates a serious difference in appearance. After this foundational support is established, if there is still some residual depression in the nasolabial groove, then a softer filler is used as a final touch in that area. This holistic approach repairs the architecture of the face and allows us to achieve a much more natural, lasting, and aesthetically balanced result.
What Are the Risks of Nasolabial Filler?
Nasolabial filler is an extremely safe procedure when performed under the right conditions by a qualified physician. However, as with all medical procedures, there are some possible side effects and, although rare, certain risks. Being informed about these helps you manage the process more consciously.
The common and temporary side effects that may be seen after the procedure are as follows:
- Mild swelling
- Bruising
- Redness
- Sensitivity in the treated area
- Temporary asymmetry
These effects usually develop due to the injection itself and disappear completely on their own within a few days to a week.
There are also some rarer complications that should be noted:
- Infection
- Allergic reaction
- Nodule formation (accumulation of filler)
- Granuloma (foreign body reaction of the body)
- Vascular occlusion
The most serious among these, although extremely rare, is vascular occlusion. This occurs when the filler material is accidentally injected into an artery and causes the blood flow to be blocked. Its symptoms include severe pain during or immediately after the procedure, sudden whitening of the skin, followed by a purple, net-like appearance. This is a medical emergency that requires immediate intervention. For this reason, it is vitally important that the physician performing the procedure has an excellent command of facial anatomy, uses safe techniques (for example, the use of a cannula), and is equipped to recognize and manage a possible complication immediately, especially with access to hyaluronidase.
What Should Be Considered After Nasolabial Filler?
The period after nasolabial filler application is just as important as the procedure itself. Following your physician’s recommendations speeds up recovery, reduces side effects, and improves the quality of the result achieved. There are some basic rules you should follow after the procedure:
- Avoid touching the area for the first few hours
- Do not wear makeup for 24 hours
- Avoid heavy exercise for 24–48 hours
- Avoid hot environments such as saunas and Turkish baths
- Apply intermittent cold compresses to the area
- Do not consume alcohol for at least 24 hours
- Sleep with your head slightly elevated
- Avoid blood-thinning medications and supplements
- Do not have facial massage or skincare treatments for two weeks
These simple precautions help the filler integrate with the tissue and allow the swelling to resolve more quickly. If you encounter an unexpected situation (severe pain, change in skin color, etc.), it is very important to contact your physician without delay.
